Appointment Checklist

If you are a new patient, complete the Patient & Health History Form and the HIPAA Authorization Form. If you plan to fill the forms out in our office, please come in 15 minutes early to handle the paperwork. (New Patients Only)

Your eye examination appointment with the doctor is scheduled for 45 minutes. Please allow 5 to 10 minutes for check out.

Your eyes may be dilated as a part of your eye examination. If dilation is necessary, expect for your vision to be blurry up close and light sensitive for up to 3 hours.

Contact Lenses

Come in wearing your contact lenses for at least 1 hour.

If you are a new patient, please bring the contact lens packaging from your contacts or a copy of your last contact lens prescription. (New Patients Only)

Keep in mind, we have in stock Acuvue 1-Day Moist, Acuvue 1-Day TruEye and Acuvue Oasys contact lenses. Most other contact lenses arrive the afternoon of the next business day if ordered by 3:00pm.

We ship annual supplies of contact lenses direct to you free of charge.


Bring all prescription glasses that you currently use including readers and sunglasses.

If you are interested in looking for glasses, we recommend that you come in ½ to 1 hour before your appointment to browse our selection.

This is the type of retail experience you look forward to sharing with others. I was hoping upon hope that the folks here could help me. I had invested in a beautiful, pricey pair of prescription sunglasses a few years back. Then, the week before my visit, I had carelessly placed them on top of my car as I pulled out of a parking space. The glasses flew off the car and hit the ground violently. The lenses were OK, but the frames were severely bent. I brought the glasses into Eye Carumba, asking about a possible repair, expecting to pay a decent price. Sophia, the Eye Carumba employee, spent about 20 minutes in their lab making adjustments, coming back and having me try them on, until they felt just right. In the end, just 30 minutes after I arrived, I had a repaired pair of Paul Smith sunglasses that felt better and sturdier than the day I purchased them. And the charge? $0. I will definitely be coming back to Eye Carumba.
